Abstract;The emergency medical care has many points to be dealt with as a system. The history of emergency medical system in Japan was summarized and then, the carrier and other various medical facilities which constitute the emergency medical system at present were explained. The basic configuration in Japan includes initial emergency medical facilities if the treatment is possible in ambulatory and secondary emergency medical facilities if hospitalization is needed and a lifesaving emergency center if they are most serious. The description was made in the following order : 1) Emergency medical facilities, 2) Carrier and pre-hospital care (24-hour emergency carrier system, helicopter, doctor car, paramedic system, standardization of medical care in pre-hospital aid, selection of hospital according to the state of the sick and wounded : Direct transfer by selected carrier),3) Emergency medical information system (Emergency medical information center : Fundamental system and flow of information, a poisoning information center : Inquiries from doctors and citizens).
