Abstract;Patients are transferred to other medical institutions, if medical care is judged to be difficult. The general flow from initial emergency medical care to transfer to the special facilities and the high order facilities and cautionary note were described, so that wrong judgment in the transfer may not be caused. The description was made in the following order: 1) Emergency medical care system in Japan (initial emergency medical care organization, second emergency medical care organization, third emergency medical care organization, lifesaving emergency center), 2) When the transfer is considered (When it is judged that the patient disease improvement is thought to be impossible in present medical institution and the possibility of improving is made by changing hospital), 3) To which medical institution the patient should be transferred (To be chosen by types of diseases or seriousness), 4) Timing of transfer (The high speed in progress of the emergency disease must be noticed.), 5) Request for transfer (Directly by telephone by doctors), 6) Importance of explanations to patients and families, 7) Methods for trasfer, 8) Attentions during the transfer (Most dangerous during the transfer, Judgement if the patient can stand up to the transfer), 9) At the end.
