Abstract;Two representative studies on glaucoma epidemiology have been conducted in Japan during past 10 years. In contrast to Western studies, both indicated an extremely high prevalence of normal-tension glaucoma (NTG) and a very low prevalence of ocular hypertension (OH). In detail, however, the figures differ considerably between the studies, the prevalence of NTG being 3.60% as reported in the Tajimi study, whereas the figure was 2.04% in the Nationwide Glaucoma Survey. Similarly, OH prevalence in the former study was given as 0.81%, in comparison to 1.37% in the latter. We compared the two studies with respect to the methodology and diagnostic procedures employed. Our results led us to conclude that there may be at least two factors causing disparity of prevalence between the studies. The Tajimi study had an irregular distribution of intraocular pressure, apparently arising from a "decision effect" at the first stage of screening, in addition to possible overestimation at optic disc assessment.
