Abstract;Von Willebrand factor (VWF) is mainly synthesized in vascular endothelial cells, and secreted as a giant multimer into the blood. Generally, the biological activity of VWF such as platelet adhesion or flocculation depends on the molecular weight, and the activity is stronger as the multimer is larger. Recently, a von Willebrand factor-cleaving protease (VWF-CP), metalloprotease ADAMTS-13, was identified. Excessive flocculation of thrombocyte is caused, when the activity of this enzyme is lowered congenitally or acquiredly. It was proven that the measurement of the activity of this enzyme is very important for diagnosis of thrombotic thrombocytopenic purpura (TTP) and decision of the treatment plan. Though as a measuring method of this enzyme, activity measurement method using VWF-CP-deficient blood plasma was devised by Furlan and Tsai et al. in 1996, the method was complicated and a measuring method at the laboratory level. Last year, a specific synthetic substrate for ADAMTS-13 was prepared, and it seems to be possible to measure this enzyme conveniently in future. It is also expected that a new knowledge will be obtained on the physiological significance of this protease or the relation with the disease states other than TTP (arterial thrombosis, etc.).
