Abstract;Prurigo pigmentosa, first reported by Nagashima et al. in 1971, is a rare inflammatory dermatosis of Un-known etiology. Lesions are typically pruritic erhythernatous papules, superseded by reticular hyperpigmentation, usually symmetrically distributed on the back, neck, and chest. Several hundred cases have been reported, mainly in Japan, and some are associated with diabetes mellitus. We report prurigo pigmentosa in 2 patients with type 1 diabetes with ketosis and high titer of IA-2 antibodies. Two women, aged 28 and 31 years, reported thirst, polyuria, and weight loss, subsequently suffering from prurigo pigmentosa. Laboratory examinations on admission showed ketosis, hyperglycemia, and decreased insulin secretion. IA-2 antibody was positive with high titer in both, although GAD antibody was negative in 1 case. In both cases, pruritic eruption improved after the start of insulin treatment. We suggest that ketosis and IA-2 autoimmunity play a potential role in prurigo pigmentosa pathogenesis. (author abst.)
