| Abstract;Acute Aortic Dissection (AAD) is one of the severe diseases with high mortality. It is important that diagnosis of AAD should be made as soon as possible. But rapid laboratory test was not available for the assessment of AAD. Recently, the significance of D-dimer assay in the patient with AAD was reported in 2 articles. Diagnositic sensitivity for diagnosis of AAD was 100% in both articles. We reported the utility in diagnosis of AAD by rapid assay of D-dimer (Cardiac D-dimer), which gives us the result at 10 minutes. In that report, the sensitivity for diagnosis of AAD was also 100%. This review showed the utility of rapid assay of D-dimer in AAD. We prospectively examined D-dimer levels by rapid assay in 29 patients with chest and/or back pain without definitive acute coronary syndrome. Twelve patients had AAD and 17 patients had no AAD. The sensitivity for diagnosis of AAD was 100%, specificity was 41%, negative predictive value was 100%, and positive predictive value was 55%, when the cut off value was defined 0.5.MU.g/ml as normal limit of assay. Thus, rapid assay of D-dimer is useful for the exclusion of AAD. (author abst.) |
