Abstract;Clinical characteristics were studied of patients with cardiopulmonary arrest following acute myocardial infarction (AMI) on arrival (CPAOA) at hospital. The subjects consisted of 70 patients (49 males and 21 females with a mean age of 69.+-.12 years) among 2,637 (1,924 males and 713 females) patients with AMI who were admitted to our hospital from 1984 to 2003. The time before arrival at our hospital was 130.+-.127 minutes. The incidence of patients with CPAOA was 2.7%. The mortality rate was 91% while the overall rate for all deaths with AMI was 17% (64/383). Cardiac arrest occurred in 35% at their homes, 11% at the outside, 11% at the hospital before arrival and 43% in the ambulance car, respectively. The incidence of patients with asystole on arrival was 71% (50 patients) and in 26 patients (52%) among them heart beats regained after cardio-pulmonary resuscitation. But all patients with asystole on arrival died except one patients. Coronary angiography was performed previously in 28 patients (40%) were and the lesions were in 8 patients at 0 VD, in 4 patients at 1 VD, in 4 patients at 2 VD and in 12 patients at 3 VD, respectively. The rate of prior myocardial infarction was 37% (26 patients). It is concluded that the prognosis of patients with CPAOA following AMI was poor and pre-hospital care is important. (author abst.)
