Abstract;A case of multiple intestinal perforations due to abdominal contusion caused by child abuse is reported. The patient was a 2-year-old boy with a history of head and femur injuries by abuse who was referred to our institute after severe peritonitis. Emergency laparotomy revealed two intestinal contusions. One was a fresh duodenal perforation due to spinal compression. The other was multiple perforations at the descending colon. Closure of the duodenal perforation, resection of the injured colon, and colostomy were performed. The colostomy was closed at a second operation 4 months later. The boy recovered from the peritonitis and was transferred to a protective institution. He seems to have recovered from his psychological damage. This is a rare and severe case of abdominal trauma, but this kind of abuse trauma may be seen at any pediatric surgical institution in Japan. (author abst.)
