| Abstract;A 65-year-old female receiving maintenance hemodialysis was admitted to our hospital because of fracture of the femoral neck. Operation was performed but because of unsatisfactory clinical course re-operation was needed. Marked anemia was persisted, therefore red blood cells (MAP) were transfused during dialysis before re-operation. During the transfusion, she complained of chilliness accompanied with low-grade fever. Soon after the MAP transfusion, she developed shock associated with marked reduction of leukocyte counts. Although intensive treatment was performed, she died of septic shock. Since MAP transfusion was suspected as a cause of the fatal accident, the MAP was submitted to bacterial culture. Gram-negative bacillus was detected from her blood and also from the residue of the MAP, and both of them were identified as Yersinia enterocolitica. The DNA types of them were also coincident, therefore it was concluded that the contaminated MAP transfusion caused fatal septic shock in this case. To our knowledge, this is the first report of fatal accident associated with MAP transfusion contaminated with Yersinia enterocolitica in Japan. We have to pay great attention to bacterial contamination of blood components. (author abst.) |
