Abstract;The glucose level in early morning hunger is 70-110mg/dl, and a value less than 50mg/dl or more than 500mg/dl is treated as a panic value. To begin with, it is confirmed that there is no mistake of the sample, and the data in time series are checked. By checking the existence of fibrins or hemolysis, it is confirmed that the sample is a proper one. In the case of a low value, a mistake in respect of sampling vessel is suspected, and in the case of a high value, sample collection from infusion line is suspected. Comparisons with the retest and the data of related items are made, and urgently reported to the clinician. The cause of hyperglycemia often encountered in daily medical care is diabetes mellitus, and especially, diabetic ketoacidosis and hyperglycemic hyperosmolar coma need urgent treatment. In the diagnosis, measurement of blood and urinary ketone body, electrolyte, renal function and arterial blood gas, in addition to blood glucose, is necessary. In most cases of hypoglycemia, the cause is the drugs for diabetic therapy, but there are rare cases of excessive insulin secretion from tumor. In the differentiation, distinction of whether in hunger or after meals, and blood insulin value, are useful.
