| Abstract;Noninvasive positive pressure ventilation (NPPV) has been used increasingly to treat patients with acute hypoxic respiratory failure. However, no randomized control trials have shown NPPV to be associated lower rates of morbidity or mortality in acute respiratory distress syndrome (ARDS) patients. In immunocompromised patients with acute respiratory failure, serious complications such as ventilator associated pneumonia, severe sepsis, and cardiogenic shock increase the mortality rate. In these patients, NPPV significantly improves the outcome, because it should reduce the complications associated with endotracheal intubation. NPPV may be effective in patients with relatively mild and early improved ARDS. On the other hand, there are many reports of failure of NPPV in patients with ARDS in acute respiratory failure. Multi-organs failure is important as a predictor of mortality in patients with ARDS. Therefore, it is important to choose artificial ventilation by intubation promptly, without persisting in NPPV, when the strict general management over a long period of time is required.
