| Abstract; Objective: To study the direct origin of the left vertebral artery from the arch of the aorta rather than its conventional origin from the ipsilateral subclavian artery. Design: An anatomic study carried out in a total of twenty four cadavers. Material and Methods: Gross dissection was carried out at the root of the neck in 24 cadavers. The origin of the left vertebral artery either from the aortic arch or the ipsilateral subclavian artery was noted. The upward course of the prevertebral segment of this vessel along with its diameter at the point of origin was determined and compared to that of the right side. Results: The possible developmental basis for the origin of the left vertebral artery directly from the aortic arch suggests the persistence of the embryonic pattern observed during fifth through seventh week of intrauterine developmental period. Conclusion: The relevance and importance of the varied origin of the left vertebral artery, in spite of its reported lower incidence, becomes important in context to present day advancements in interventional procedures and endovascular manoeuvres applicable in the head and neck region.
