| Abstract;Left ventricular restoration (LVR) for dilated cardiomyopathy (DCM) was studied. Among 250 patients who received LVR, non-ischemic DCM (NIDCM) was in 126 and ischemic DCM (IDCM) was in 124. Before operation, left ventricular function and form were examined and operative procedure for LVR was selected. After the operation, in ICU, several medications were intravenously administrated for one week and medical treatment was continued after discharge. In NIDCM, mitral surgery was in 120 and tricuspid surgery was in 73 as combined operation, and IABP was used in 25 and LVAD was in 4 patients. 102 patients were discharged from hospital and 61 of them (61%) improved to NYHA class I-II. In IDCM, CABG was concomitantly performed in 91, mitral surgery in 53, and tricuspid surgery in 25. IABP was used in 18 and LVD was in one patients. 114 patients were discharged from the hospital and 81 of them (70%) improved to NYHA I-II. The LVR is useful procedure for DCM and early operation is recommended if there is indication for LVR, and then the medical treatment can be effective after the operation. (author abst.) |
