Abstract;Objective: To validate of the Malay version brief patient health questionnaire (PHQ-9) before it can be used for screening of depression among adult attending primary health care services. Design: A cross-sectional study. Methods: A study involving two hundred and sixty five patient attending family medicine clinic on August 2001 to July 2003. They were completed the Malay version of PHQ-9. About hundred and eighty participants were then interviewed by psychiatrist and they were then assessed with Hamilton Depression Rating Scale (HDRS) and Hospital anxiety depression scale (HADS). Psychiatric diagnosis was made based on the Composite International Diagnostic Interview (CIDI). The validity of PHQ-9 was tested against this clinical diagnosis and the concurrent validity against HDRS, HADS and the Malay version of PHQ-9 score was also evaluated. Results: The internal reliability of PHQ-9 is good with a combachs (alpa) of 0.67. Test-retest reliability was also good with alpa of 0.73. The best cut-off score of the Malay version of PHQ-9 was 5 with the sensitivity of 69.0% and specificity of 60.5%. The best cut-off score for severe depression was 10 with sensitivity of 60.9%, specificity of 80.7% with positive predictive value of 38.6%. Conclusion: The Malay version of PHQ-9 is a valid and reliable screening tool for detection of depression in primary care. However the sensitivity and specificity of this tool is quite low and this may be related to the different understanding and cultural variation of depression between Kelantanase population and Western population.
