| Abstract;Objective: We compared influence on blood pressure and uric acid metabolism between hypertensive patients with and without hyperuricemia by concomitant administration of losartan and low-dose hydrochlorothiazide. Methods: For 21 outpatients with hypertension (an average of 66 years old, 13 males) without medical history of gout and urinary calculus, blood pressure measurements, blood sampling and urine collection were performed every one month before and after 12.5 mg of hydrochlorothiazide concomitant use with losartan (50 mg). Results: Both systolic and diastolic blood pressure decreased significantly (P<0.01; p<0.05, respectively) by use of losartan and hydrochlorothiazide. The uric acid/the creatinine clearance ratio increased significantly (p<0.05), while the serum urate concentration revealed 6.7.+-.1.5 mg/dL and 6.5.+-.1.6 mg/dL, before and after administration, respectively. Both in candidates with and without hyperuricemia (n=11 and n=10, respectively), the serum urate concentration revealed unchanged. The uric acid/the creatinine clearance ratio increased significantly in candidates with hyperuricemia. Conclusion: Concomitant use of losartan and low-dose hydrochlorothiazide might induce an inhibitory action on URAT1 in hypertensives with or without hyperuricemia to exert both lowering blood pressure.
