| Abstract;Backgroud and Method: It is unclear whether the response to antiarrhythmic drugs therapy (AAT) in patients with paroxysmal atrial fibrillation (AF) affects structural remodeling in left atrium and ventriculum. This study included 230 patients (158 men, 72 women, mean age 67.+-.11 years) in whom AAT was attempted for .GEQ.12 months to maintain sinus rhythm in the long-term (mean follow-up 45.+-.27 months). The patients were divided into the 3 groups; The patients without recurrence of AF (A-group), those with recurrence of AF and with electrical and pharmacological cardioversion to restore sinus rhythm (B-group), and those with conversion to permanent AF (C-group, n=65) despite AAT. Results: (1) The patient's group treated with disopyramide (n=78) and cibenzoline (n=77); In group-A, there was no significant difference in the left atrial dimension (LAD), left ventricular end-diastolic dimension (LVDd), and left ventricular ejection fraction (LVEF) between before and after AAT. In group-B, LAD after AAT was significantly higher than that before AAT (p<0.01), whereas there was no significant difference in LVDd and LVEF between before and after AAT. In C-group, LAD after AAT was significantly higher than that before AAT (p<0.01), and LVEF after AAT was significantly lower than that before AAT (p<0.05). However, there was no significant difference in LVDd between before and after AAT. (2) The patient's group treated with aprindine (n=75); The changes of LVDd, LAD and LVEF before and after AAT in A and B group were consistent to those in the patients treated with disopyramide and cibenzoline, whereas there was no significant difference in LVEF between before and after AAT in C group. Conclusion: The development of structural remodeling in human myocardium may be prevented with AAT if sinus rhythm is maintained without recurrence of AF in patients with paroxysmal AF....
