| Abstract;The roles of calcium channel blockers used to treat renal diseases in combination with other drugs are discussed in brief. Blood pressure management is important in the treatment of renal diseases. The Japanese Society of Hypertension Guidelines for the Management of Hypertension 2004 (JSH2004) does not recommend calcium channel blockers as the first-line treatment. Considering their antihypertensive effect and tolerability, however, they can be selected as the first-line drugs. Certain calcium channel blockers are reported to have a renal protective effect by dilating the efferent arterioles. Although the results of a large-scale clinical study to investigate their effect of preventing the progression of renal damages are controversial, they are often used, owing to stable antihypertensive effect, in combination with renin-angiotensin system (RAS) inhibitors in the clinical setting. We used benidipine in combination with valsartan in our patients. This combination therapy normalized blood pressure and urine protein in some of them. The large-scale clinical study suggested the possible superiority of calcium channel blockers to RAS inhibitors in primary prevention of stroke occurring with a high frequency among the Japanese. Calcium channel blockers may be useful to inhibit the onset of cardiovascular diseases during the course of long-term treatment of renal diseases. (author abst.) |
