Abstract;Pioglitazone is an insulin sensitizer of thiazolidinedione (TZD) class. To analyze its effect on serum fasting insulin (FIRI) levels, the following studies were performed. Newly diagnosed, drug naieve patients with type 2 diabetes (n = 41, average age: 53.6 years old, female/male = 15/26) received 15 or 30 mg/day pioglitazone. At 3 months, the levels of FIRI and HbA1c were compared with those at baseline. Significant reductions were observed with FIRI (11.07.+-.8.86 .MU.U/mL vs. 15.50.+-.13.07 .MU.U/mL, p = 0.031, n = 41, Fig. 1A) or HbA1c levels (7.61.+-.1.94% vs. 9.28.+-.2.22%, p < 0.0001, n = 41, Fig. 1B). Then the patients were divided into two groups; those with high baseline FIRI (above 15 .MU.U/mL, n = 23) and others with low baseline FIRI (below 8 .MU.U/mL, n = 14) levels. In the high FIRI group, significant reductions of FIRI (12.26.+-.9.20 .MU.U/mL vs. 21.56.+-.12.66 .MU.U/mL, p = 0.0009, n = 23, Fig. 2A) and HbA1c (7.21.+-.1.89% vs. 8.86.+-.2.22%, p < 0.0001, n = 23, Fig. 2B) levels were observed. In contrast to this, in the low FIRI group, significant increases of FIRI (8.97.+-.8.10 .MU.U/mL vs. 4.67.+-.2.50 .MU.U/mL, p = 0.025, n = 14, Fig. 3A) and decreases of HbA1c (8.28.+-.1.82% vs. 9.88.+-.2.08%, p = 0.00014, n = 14, Fig. 3B) levels were observed. It is of note that baseline HbA1c levels were higher in the low FIRI subjects than those in the high FIRI subjects (9.88.+-.2.08 vs. 8.86.+-.2.22, p < 0.05). These results suggest that pioglitazone is very effective as a first-line drug in newly diagnosed patients with type 2 diabetes and its effect may be mediated through increased insulin levels as well as decreased insulin resistance, depending on the baseline insulin values.
