Abstract;We report toe web infection in a 55-year-old man (case 1) and a 73-year-old man (case 2), neither of whom had diabetes. The patients had been treated with topical antifungal agents without direct microscopic examination of KOH preparations. When the patients presented at our department, a foul smell and green effusion accompanied erosion and ulceration of the toe web. The patients had difficulty in walking because of a throbbing pain. Specimens from the infected area were negative in the KOH direct microscopic examination, and Pseudomonas aerugiosa was cultured on them. In case 1, topical antibiotic ointment and topical anticulcerant were not effective, but intravenous antibiotic was effective. In case 2, intravenous antibiotic and topical silver sulfadiazine were successfully administered from the start. In these cases, eradication of the bacteria which caused the infection was essential and early administration of systemic antibiotic was effective. (author abst.)
