Abstract;An outbreak of acute encephalopathy has occurred among forty-one patients with renal dysfunction after ingestion of "sugihiratake" mushroom in the northern area of Japan between the end of September and the middle of October, 2004. Thirty-eight patients had varying degree of renal dysfunction, of which twenty-eight were on dialysis. Patients' initial symptoms were asthenia in the legs, difficulty in ambulation, and tremor-like involuntary movements. Within several days, mild disturbance of consciousness, and myoclonus developed, followed by generalized convulsions, which frequently progressed to status epileptics. Eleven patients died. CSF examination revealed elevated protein without pleocytosis. Brain CT and MRI studies showed abnormal intensities in external capsules, and in the subcortical white matter. Bacterial and virological investigations have been negative. All of the patients had a history to have ingested "sugihiratake" mushroom (angel's wings mushroom, pleurocybella porrigens) prior to the onset. No similar cases have been reported after ingestion of this mushroom. The pathomechanisms of this encephalopathy remain to be elucidated. (author abst.)
