| Abstract;We treated a 25-year-old man with central nervous system sarcoidosis who presented with meningoencephalitis. The patient had a fattv liver with slight elevations of hepatic enzyme levels. His meningoencephalitis was successfully treated using a high dose of corticosteroids (i.e., a 3-day consecutive intravenous administration of methylprednisolone at a dose of 1,000mg/day, followed by the oral administration of prednisolone (PSL) at a dose of 60mg/day). However, PSL dose tapering resulted in the relapse of the meningoencephalitis. A supplemental oral administration of methotrexate (MTX)(single dose of 10mg/week) allowed the PSL dose to be reduced to a level of 5mg/day without triggering a relapse. At present, the cumulative MTX dose is about 3,000mg, but no adverse effects from MTX have been encountered. We propose that lowdose MTX, administered as a single dose of 10mg/week, may be useful as a corticosteroidsparing agent in the treatment of CNS sarcoidosis. (author abst.) |
