Abstract;Clinical features and prognosis of heart failure with preserved systolic function, or diastolic heart failure (DHF), remain unclear in Japan. In our analysis in 394 patients with decompensated heart failure (NYHA III-IV), 124 patients (31.5%) were diagnosed as having DHF. Advanced age, female gender, elevated s-BP on admission and absence of history of CAD were the four major clinical factors that strongly suggest the presence of DHF. After hospital discharge, DHF had a similar mortality rate but a lower rehospitalization rate compared with SHF. Because of the paucity of clinical evidence for the treatment of DHF, present therapeutic guidelines are not clear-cut. Some suggestions for the treatment of DHF are presented. (author abst.)
