Abstract;I reviewed clinical trials on the treatment of dementia of Alzheimer type (DAT) and vascular dementia. Dementia of Alzheimer type (DAT): Acetylcholine esterase inhibitor (AchEI) has been shown to have beneficial effect in cognitive decline of mild to moderate DAT, but it was not clear whether it is beneficial in severe DAT. In a clinical trial of donepezil in moderate to severe DAT, the mean scores on Clinician's Interview-Based Impression of Change (CIBIC-plus) in the donepezil group were significantly improved than those in the placebo groups. Thus, donepezil therapy was suggested to be effective for moderate to severe DAT, as well as mild DAT. Mild cognitive impairment (MCI) has been converted frequently into DAT. A trial of vitamin E and donepezil for the treatment of MCI was performed. As a result, there were no significant differences in the rate of progression to DAT between the vitamin E and placebo groups, but the donepezil group showed lower rate of progression to DAT during the first 12 months of treatment. Other open-label studies suggested that treatment of rivastigmine and galantamine were shown to be protective against the cognitive decline assessed by scores on Neuropsychiatric Inventry-nursing home (NPI-NH) or attention in patients with DAT. Vascular dementia (VaD): To assess a benefit of donepezil for VaD, combined analysis of 2 randomized, double-blind, placebo-controlled studies was performed in 1,219 patients with VaD who were enrolled in several countries. Donepezil group respectively showed significant improvements in mean scores on Alzheimer's disease assessment scale-cognitive subscale (ADAS-cog), mini-mental state examination (MMSE), CIBIC-plus and Clinical Dementia Rating-Sum of the Boxes (CDR-SB) compared with the placebo group. Thus, authors reported that donepezil might be an effective agent in the treatment of VaD. (author abst.)
