Abstract; The case was in a 19-year-old woman, and many red papules and pimples occured coinciding with hair pores in both outside buccal region. Usual minocycline internal use and antibacterial agent external use were performed over 2 years by a regional doctor, but remission and aggravation were repeated. Since trichauxis and irregularity of menstruation were recognized other than acne pustlosa as a diagnosis, androgenic acne by polycystic ovary syndrome (PCOS) was also differentiated. Internal use of minocycline and clindamycin gel external use were started without effect, and 3 tablet/day of diol, a corpus luteum hormone metabolite, was prescribed additionally. Since there was no change in the symptom after 1 month, diol was supended, and changed to memolsane-F, a comprehensive metabolic hormone drug, by 3 tablets/day. However, since there was no improvement, memolsane-F was stopped after 3 months, and changed to ortho M21 of low dose pill. Papules and pustules were improved after 3 months of ortho M21 administration. However, since the hairy symptom was not improved, the diagnosis of PCOS was asked for gynaecologists, and it was decided by echography and endocrine diagnosis as PCOS, and the treatment by fertility drugs is continued. The PCOS is with the 3 symptoms of obesity, anovulation and masculinization, but the most of the PCOS patients in Japan are with mensual abnormality only. Diol is an urinary metabolite of progesterone, and it has been said that there is no hormonal effect. However, this drug seems to show slight progesterone action.
