| Abstract;The case was in a 33-year-old man, nodes and subcutaneous indurations were felt on the face, precordia and back, and intractable acne with abscess and fistula was recognized. Pigmentations and cicatrices intermingled fromm place to place. Some parts were necrotized and crusts were produced. It was diagnosed as convergent acne, because lesion was not recognized in axillae or buttock. In the initial examination, the subcutaneous abscess was drained of pus by opening with carbon dioxide laser, the internal use of antimicrobial drug was started, and external use of tretinoin was started from the next day. When subcutaneous abscess was produced, the pus was removed every time by carbon dioxide laser. The secretion of sebum was suppressed gradually, and acne was sedated by the treatment for about 3 months. The treatment of the acne is considered to be divided into 1) the treatment for the exsisting acne, and 2) the treatment to suppresss the generation of new acne, and they should be used jointly according to the necessity. This is the same also in the treatment of convergent acne. Because abnormality of pilosebaceous system or the constitutional factor for bacterial infection of individual organism seems to be one of the causes of the disease, suppressipn of sebum secretion and administration of antimicrobial drug seem to be rational treatment. In men, since side effects such as gynaecomastia may be produced by spironolactone, the internal use of isotretinoin is recommended. Isotretinoin is a kind of retinoic acid with powerful suppressive effect for sebaceous gland function and dyskeratosis modification effect, and exact contraception is necessary, but in men, it is comparatively easy to use. When the patient hesitates for internal-dose treatment, both for man and woman, topical tretinoin and chemical peeling are the alternatives. |
